The MINISTRY of health has made changes in the procedure of choosing a doctor who provides primary medical care, in accordance with which now to the signing of the Declaration with the doctor copies of documents no longer needed. About it reports a press-service of the Ministry of health, informs Rus.Media.

“The doctor or the Registrar is no longer obliged to make and retain copies of documents provided by the patient,” – noted in the MINISTRY of health.

Now the patient in signing the Declaration should only show one of the identity documents and the document on assignment of registration number of accounting card of the taxpayer.

Now we need to store only one copy of the Declaration signed by the patient. Another instance, as before, remains in the patient.

In addition, the MINISTRY of health has simplified these forms – now they are on a single sheet, saving the time of patients and the doctor filling it in.

“The Declaration now do not need to specify the registered place of residence of the patient, since the patient chooses a physician, regardless of place of residence”, – stressed in the MINISTRY of health.

Also extended the validity of the declarations.

Until January 1, 2020, extended the validity of a Declaration of choice of doctor, which were signed prior to March 23, 2018.

The Declaration entered into after March 23, 2018, have a finite duration. They do not need to renew.

Also, now remain valid Declaration after the change of legal entity of the institution.

Recall that more than 10 million Ukrainians already chose my doctors and concluded with them contracts on rendering of services.

The campaign “Doctor for each family” was launched in Ukraine on April 2.

As of 27 June on the website of the electronic system e-Health was registered 1 262 thousand medical institutions, 98 22 thousand primary care physicians and 10 million 341 thousand 520 patients who already have drawn up the Declaration.
